Missing extensions?

I need to update an add on but when I go to Tools>Extensions it's blank. I've used this to update extensions many times without issue so I'm don't know what would have changed. Any suggestions?

I don't see a dropdown in the Extension window. It looks the same as always but there are no extensions listed and most of the buttons are grayed out.

Over in the add on thread for the Discogs tagger it was suggested I open MM4 as an admin. I don't think I've ever done that and don't know how, but would that help?
