I've not seen this before today and I'm not sure what, if anything, has changed.
When I rip a CD, the Rip dialog appears behind the main window. I only found this out after having to kill MM a few times because it became unresponsive with a darkened window. I finally realised what might be happening and used MM in a non-maximized window and now I could see the CD Rip window, move it to the front, and use it.
Update: after ripping one CD, I popped another in the drive and this time the CD Rip window appeared in front of the main window.

I have never seen this, is this still reproducible?
Or was it a one time issue?
I guess that there must have been some specific steps (e.g. interactions with other windows) to get into this state?

I have seen it once or twice (not recently) and ALT+TAB always brought hidden window on top.
