Track volume spikes to SUPER LOUD at the very beginning of tracks. Like, the first quarter-second of each track -- it's very short and very loud. This happens when automatically transitioning to the next song in the playlist, or when you skip to the next song, or when starting a track from a fresh launch of the player.
My collection is primarily MP3s encoded between 160-320 bits, and I don't convert when copying to my device.
This doesn't happen with other audio players on the device.

Do you have gapless or crossfading, or volume leveling enabled in MMA?

Steps to reliably replicate the issue:
1. Launch MMA
2. Find the track and play it for a few seconds
3. Stop the track and put your device to sleep for at least an hour
4. Wake your device, launch MMA, and resume playing the track where you left off in Step 2
That's how to reliably replicate the loud spike in volume. I'm using wired headphones -- I haven't tested using a Bluetooth speaker or the device's speaker.
Since opening this thread, I replaced all the tracks and playlists on my device and I don't experience the loud spike at the beginning of songs as frequently as I used to.

Sorry for the delay. I'm not being able to replicate with or without headphones, but I noticed that after the received call sometimes Volume was left on 100% for a split of second on play resume. Unfortunately that looks like Android issue rather than MMA
