I'm not sure if this is possible for the MMv4 or not, but here is what I plan on doing.
My music library is stored in my dedicated desktop at home. I want to be able to access my music using my laptop, which is either connected to my local network or to my work network. I was able to access my library on local network using the UPnP server feature, but I'm having troubles accessing my library when connected to my work network.
I'm using port 4000 on my UPnP server and have port forwarded successfully. I also tried disabling my firewall.
To connect to my desktop server at home, on my laptop's MMv4 I go to File > Open URL > and type http://<ip>:4000/1, but nothing happens.

I don't think MediaMonkey can open a DLNA Server from outside of the network. There are clients that can though, often smart-phone clients, but you'll need to research which.
To verify that you've setup things correctly use a browser on the work PC to navigate to http://<ip>:4000 and you should see an XML file returned. If that works you can look for a client that can connect to an off-network DLNA server.

DLNA is based on UPnP(-AV) which does not support access from outside the LAN.I'm having troubles accessing my library when connected to my work network.

Some media servers like Twonky provide some sort of remote access. For obvious reasons, however, they don't use DLNA but a built-in web server.
Why not use a service like iTunes Match or Google Music?
