Ok it looks that it only happen when tracks are shown in Library tree Node and with specific settings. Here is my settings in Tools -> Options: 1. Player Double click action -> Play now -> Clear playlist & Play selected tracks + subsequent 2. Library -> Appearance -> Show tracks in Library node...

Second Update:

Decided to uninstall MM and erased all files left behind. I also cleaned the registry.

UPDATE: This issue only occurs when I play a song after starting MM for the first time. When I start playing other songs there is no issue. But, if i close MM it will happen again. Are you starting MM with Now Playing populated or do you have a script installed that clears the now playing on exit? ...
