It might be that you need to specify that the playlists get synced even though you're already synching all the files that are already in those playlists. I personally don't have MM do my entire library, so I don't know.

Did you tell MM to sync your playlists? I'm not in front of my MM PC right now, btu it's in the sync options for your device. You can specify which playlists to sync. Don't know if you need to do this only if sync part of your collection, not the whole thing.

Try rebuilding the device database. That usually clears up album art problems for me. Right-click on your device in the folder tree and you'll see the option.
