I cannot replicate, the custom headings are transferred in my case.
They are stored under [CustomFields] section in MediaMonkey.ini file and the file is transferred alson with MM.DB from MM4 to MM5
I tested this way:
- standard install of MM4 with 'Custom 1' field set to 'My Custom 1'
- installed MM5 portable and accepted the prompt to copy settings and database from MM4
- the 'My Custom 1' heading was transferred:

BUT, in my case the database was empty! My MM4 database has thousands of track, but the MM5 is empty with no tracks!!
=> will be fixed in 2167
Could you check whether you have only on MM4 install and only one MediaMonkey.ini + to look into the MediaMonkey.ini whether the [CustomFields] section is there with correct values? Maybe you installed MM4 in the past, then you installed MM4 as portable and thus has two MM4 versions and settings on the same PC ??
